Bloodborne Pathogens Exposure Control Plan
Hepatitis B Vaccination Q & A

Hepatitis B Vaccination Declaration Statement Form

OSHA 29CFR1910.1030 "Bloodborne Pathogens"

Bloodborne Pathogens

The University Bloodborne Pathogens Exposure Control Plan (ECP) contains general requirements for practices and procedures that will protect employees from the hazards of exposure to blood or other potentially infectious materials (OPIM). Environmental Health and Safety has developed an Exposure Control Plan to be used in developing departmental or site-specific programs.

EHS has identified job classifications and job dutires which are considered to pose a risk of occupational exposure to bloodborne pathogens. These classifications can be found in Appendix A of the Exposure Control Plan. Each employee who is classified as having a reasonably anticipated exposure to bloodborne pathogens or OPIM must complete the following:

  • Annual BBP training which consists of OSHA-required and site-specific information, such as Universal Precautions, communication of hazards to employees, post-exposure follow-up, etc.;
  • Hepatitis B Vaccination Declaration Statement form within 10 working days of initial assignment; and
  • Follow-up with supervisor and Human Resources Workers Compenstation should an occupational exposure occur.

Please reference the University Exposure Control Plan for a more detailed description of site-specific policies and procedures.
