Non-Employee Accident Reporting
Non-Employee Accident Reports are used following the occurrence of an accident involving a non-employee on university grounds or in university buildings. University Risk Management will maintain these records and will investigate accidents if deemed necessary. If you are not an employee of Illinois State University and would like to submit an accident report for yourself or in-lieu-of another party, please complete the Non-Employee Accident Report form which can also be found on the University Risk Management Webpage.
