Non-Employee Accident Reports are used in the occurrence of an accident on campus with a non-employee. The Risk Management Office will investigate accidents if deemed necessary. If you are not an employee of Illinois State University and would like to submit an accident report, please complete the Non-Employee Accident Report form and submit it through campus mail (Box 1270) or USPS to: Campus Box 1270, Normal, IL 61790-1270. You may also submit a form via email to riskmanagement@ilstu.edu.
